Use this service to submit a formal complaint to the surgery online.

We take complaints seriously and use them to improve the care and service we provide. This form allows you to raise a concern if you are unhappy with any aspect of the care or service you have received from the surgery. If you haven’t already please read our complaints procedure before submitting.

If you are submitting this form on behalf of someone else who is over 16, a third-party consent form will be sent to the email address you provide upon receipt of this form.

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• details of the complaint, including what happened and when
• if applicable, the details of the person you are completing the form on behalf of
