Long Term Condition ReviewWhy have I been Invited?

If you live with a long-term health condition you have likely been directed to this page. Our Practice will invite patients annually who have the following conditions:

  • Asthma
  • Cardiovascular Disease/ Heart Failure (Atrial Fibrillation, Chronic Heart Disease, Hypertension & Peripheral Arterial Disease)
  • COPD
  • Dementia
  • Diabetes
  • Learning disabilities
  • Mental Health
  • Stroke or Transient Ischemic Attack

Long term condition review – What is it?

This annual review is ran from April to March, where you will be recalled based on an internal system, this means you will likely be invited at the same time each year. However, if you have a preference on when you would like this to be, please let us know so we can make the necessary amendment. The idea behind this system is to reduce the amount of time you may need to attend the surgery and to complete all your checks within 1 or 2 visits.

The Process:

Step 1: You will receive an invite

This will be via text, email or phone depending on your chosen preference.

In the messages, there will be a link allowing you to book directly into an appointment for the Annual Review.

Step 2: Initial Appointment – Physical Checks

This will be an appointment with our Health Care Assistance (HCA) where you may be asked to provide a blood or urine test and they will check other key readings such as blood pressure, height and weight.

If you just require a blood test you will only receive an appointment request with our Phlebotomist.

Step 3: Follow-up

Depending on what Long-term condition you have, the HCA will either inform the relevant clinician to review the tests taken or book you in with them directly to discuss.

COPD or Asthma invites

If you have COPD or Asthma you will receive a separate invitation to complete a questionnaire online. Once you have completed the questionnaire, your responses will be sent to our clinical team. They will review your answers and decide if you need to have a further discussion with them via an appointment.

Alternatively, if you have any troubles completing the questionnaire or would prefer to speak to someone directly, please call reception and ask for an ‘Asthma Review’ appointment.

Long term condition review – Why it matters?

This review helps you and your healthcare team make sure your condition is well managed and that you’re getting the best care and support. Even if you are feeling well, these reviews are important because:

✅ Detect problems early
✅ Review your medication safely
✅ Reduce the risk of complications or hospital admissions
✅ Plan your care for the year ahead
