Registering with the surgery
You can register with the surgery in the following ways:
- Online Registration: Register Online
- Paper Form: Download and complete the registration form.
Please download and complete the registration forms and GMS1 below and bring them to the surgery with 2 forms of ID (address and photo ID).
- New Patient Registration (Over 16s)
- New Patient Registration (Under 16s)
- Family Doctors Surgery Registration Form (GMS1)
- Your Summary Care Record (SCR) and your Summary Care Record with Additional Information (SCRAI) form
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
